Quite a good video. I would like to use it for patient ed.
The Joint Principles start with: Personal Physician and Physician Directed Medical Team. The word physician is never heard in this video and the image of the Personal Healtcare Provider appears as a nurse. Many physicians will find this video inadequate. I recommend replacing "Personal Healtcare Provider" with "Your personal physician, nurse practitioner or physician assistant." It is a little more complicated but necessarily so.
