Backwinding could cause damage to carp if its used the wrong way, YES.
If you allow the line to go slack, i.e not keeping contact with the fish by pulling back on your rod when backwinding, there's a chance your mainline can get teased between the carps scale and its skin. It is quite rare though for this to happen since most anglers today use rig-tubing,.
