There's been hints toward Pixel -thinking- about making a second game. But if it were ever to happen, he would mostly want to do the work on his own to make it feel his own way unlike other games. Pixel started having second thoughts about a second game after the game was more noticed as it got on the 3DS. But it could easily not happen. But if it was, I'd want Pixel to be in complete control of the project. And if it does happen, I hope he does it on his own time and not on a companies.

@SirJamster There's a (possibly sarcastic) remark made at the end of the game (forget by who) that they would definitely not see the Doctor a second and third time. I think this is a pretty obvious allusion to what could be Cave Story 2 and 3 at some point down the line.
