http://spanair.airsafe.org - Dr. Todd Curtis reviews the preliminary findings in the investigation of the 20 August 2008 crash of a Spanair MD82 in Madrid. Spanish investigators reportedly found that the flaps were not properly configured for takeoff and that the crew flap configuration alarm was not activated. A video taken by the Spanish airport authorities also show that there was no indication of a fire or explosion while the aircraft was airborne.
This kind of pilot error seems relatively common after some kind of delay. There was another crash similar to this one where checks weren't made, and in that case too the pilots had been delayed and their runway changed at the last minute. After delays of this sort there should be some kind of change in the proceedure that recognises the possibility that the pilots may now be slightly agitated and prone to distraction.

Thats was sad and the plane was cheked but they cut of the system thats was the problem :(

hey pilots, you have to check,check, and check.....wake up!! that plane wants to crash more than it wants to fly. i can understand something out of your control....pilot error?, come on!
