This is not perfect but it was hard to make.
It's based on a fic.

The Doctor meets a tiny blonde girl not knowing she is in fact Rose's daughter. She has to help her to go back to her world, but he doesn't know there is no world to come back, when he realizes he must travel in time and to other dimensions to save her.

PS: The reunion in season 4 never happened in my fic
