This video presentation describes two patients who consulted with Kenneth J. Rosenthal, MD FACS, (Surgeon Director, Rosenthal Eye Surgery, New York, and Great Neck, NY) after having sustained trauma to the iris (colored part of the eye) during what was otherwise routine cataract surgery. This video is intended for the professional opthhalmic community but lay public is welcome to view it (warning!: explicit eye surgery footage is included.

  • can i ask a dumb question? why is the iris implant white? why dont they make a blue and a brown? or at least just a brown?

  • it is actually a light blue, just looks more pale under the bright lights of the surgical microscope. Also available in brown and green.

  • great video! Jsut one observation that I did not see you perform anterior vitrectomy in the second patient in whom the posterior capsule was open and you commented that the PCIOL had to be removed.... are these devices comercially available? Dr Ankur Agarwal MD FRCS Consultant Retina Surgeon

  • I did vit but didn't show entire case, only parts pertinent to subject. They are commercially available outside the US but in the US not FDA approved. We obtain them through special single compassionate use exemption.

  • thanks for the great viseo. What measurements did you use when determining how far posterior to the limbus you went when placing the scleral sutures for iridodialysis repair?

  • I don't remember in this case, but today we place them by using intraoperative endoscopy and looking to see that the suture are placed in the ciliary sulcus.

  • hello I wanted to know if this can be done in the eye because I'm interested because I have this problem practically of these gentlemen. I live in Spain wanted to know if this is done in the U.S. for me to go .. is reliable

  • Steve, I place sutures based on direct endoscopic observation. I aim for a place just anterior to the ciliary body (in what would be, with an iris, the "ciliary sulcus"). This is generally about 2.5 to 3mm behind the limbus but I find the limbus an unreliable landmark in every case.
