@bobbydj01 It's the cap to the mouthpiece. Because the entire mouthpiece is part of the reed and, if broken, can't simply be replaced like the reeds of an oboe or basssoon, the cap is there to protect the wood from being damaged. I'm guessing that because it's an essential part to protecting the instrument, they have connected it to a string so that it is never lost.
