I ordered this from Hong Kong and had it delivered in 2 weeks to Perth Australia.
It seemed not to work when I upgraded my iPhone to 4S but probably that was just because I had too many things going on with new phone, upgrade OS etc. So take it out and upgrade your phone without it. When all is working again, then reinstall the Q-sim.

I have used this product all over the world. It is flimsy and you will need to push it a little in case it jams or creases. When I travel I use this and just purchase a data/talk prepaid sim wherever I am. Usually the cost is about $50 per month.

can you receive calls from both sims, like a dual sim phone, so when you get a call it will say which sim its coming from?

@dragass With Divert function in your iPhone 4 "YES" you can use both sims in your iPhone 4 with this DUAL SIM adapter like a dual sim, there is no dual standby sim
