@WalterLiddy I think it was to test if you had any ruptures or hernias in the pelvic floor. If you coughed it would come through and would be felt as a knot or bulge in the region of your scrotum. I don't know if they still do it or not. But they did at one time.

This episode explains why Jonathn Creek is so brainy. It looks like he comes from a highly educated brainy family, his mother is 'Dr. Sally Creek'. Unfortunately, he is working as a magician's assistant, with that kind of intelligance he should have been a professor.
