If you want this difficult yet fully obtainable achievement you can edit the registry value:
HKEY_CURRENT_USER\Software\CD Projekt RED\The Witcher 2
When you die, the game puts a bogus value to that registry key. This value will make you unable to access your previous saves. If you want to resume your game, you will have to edit that registry value back to what it was before you died.
There is a utility that will monitor that registry value for changes. It will lose those values if you EXIT out of the game. So you must keep it open when you want to Restore a value.

Some notes to Insane Mode:
- Fighting the Operator --- AVOID Do not say you are not chosen. The fight is near impossible in this difficulty. Its possible to do it, but killing him for the items is not really worth it.
- Going to Iorveths path will allow your character to reach level 35. Going Roche's path cap at 32.
- You can skip a majority of the prologue by selecting different dialog options with Roche
