@chrisgrinhard No offence guys, but how about PHYSICALLY experimenting with tactics before concluding what works and what doesn't? Get yourself a blunt plastic knife (sharkee are good) - not rubber because it bends and not aluminium because it's too hard. Get some eye protection for the 'defender' and some headgear for the knife attacker... then just go for it. All-out, full force. if you believe that him grabbing you is pointless to fight then you've already given the knife attacker power over you.

a 10 inch knife is scary, but I'd go for his vitals and strike to kill and barrage him until he stops breathing, not giveing him any time to recuperate between my kicking his groin,fingering his eyes, grabbing his arm(arm holding knife), punching his throat REPEATEDLY FULL FORCE.

of course if he grab you, you WILL get stabbed/slashed, but if you POP HIM GOOD WITH ALL YOUR MIGHT IN HIS THROAT...HE IS DEAD.
NEVER UNDERESTIMATE THE EFFECTIVENESS OF STRIKING VITAL AREAS.
I don't think ignoring the knife holding arm and instead trying to punch or kick the attacker is a possible or sensible thing to do. No matter how much damage or pain you can inflict on the attcker, the attacker will inflict more fatal damage on you in return with his knife. So first thing first you will be left with no choice but to disarm that knife first by defending yourself against it. In fact, defending yourself from the knife is a challenge in itself let alone disarming that weapon.
@tacprotection no offense taken. I respect what you're saying dude, but I HAVE ACTUALLY DONE KNIFE/GUN DEFENSE TRAINING.

yup, and since most deaths by knife attacks take about 10/80 stabs you have a good chance of making it out alive.
keep distance and try and defend your main blood vessels, the neck legs and stumach.
carefull about the lungs too!
