From what it looks like to me is the ball carrier as hit by guy 1, and was being taken down, guy two comes it to assist and comes from behind guy 1, their helmets collide, smashing guy 1's helmet into the carrier's helmet. I think the facemask might have caught on the bottom of it, yanked it off, and then just happened to fall onto the carrier's head...backwards.
