I kindly disagree. People were still cool in those days, and this sort of humor was more present. These days, we've lost all out subtlety, just watch an axe commercial.

I do not think that the line was intended as innuendo. We, in the "modern era," are reading too much into it. As for Julia's reaction, she knows that Richard is trying to undermine her plan to take Annette to Mackinac Island to live. Richard is reinforcing the idea that, with him, she will be able to live like a Queen, whereas in Mackinac, she will be just plain ordinary folks. Annette is too conceited to stand for that!
