yes , i had an open anterior inguinal approach without mesh . my pain scale now is 6 or 7 .

hi i am from egypt . i had an operation like this 4 years ago . after i had this operation i felt avery weird pain . i live with this pain untill now .(chronic pain) .
after i had the hernia operation i had another operation (Laparoscopic operation) to descover source of the pain

egyptian surgeons found that my abdomial wall in the back ( not the one infront ) r came up and covred most of the spermatic cord or some thing like that . they did nothing . i will upload Laparoscopic video later . i want 2 know what is your opinion ? surgeons did alot of serches on net . they didn't find any match for my case .
I assume the deep layer of mesh spontaneously un-'crumples' (flattens) during or shortly after the procedure. Nevertheless it looks a bit frightening to watch what appears to be a wad of compressed mesh poked into the peritoneum. Also, it appears the mesh was cut in-situ to to accommodate the spermatic cord. Is this correct?

Yes it is.
The key point is the gently dissection with a gauze of the preperitoneal space (Bogros space), to actualize the needed space to insert the underlay portion of the mesh and its deploy as best is possible with a finger. After the trasversalis fascia has been closed around the connector, a lateral slit to accomodate the cord, has been made on the onlay mesh

Your operation (hernia repair), you had 4 years ago was an open anterior inguinal approach? A PHS prosthesis was used? in a VAS pain scale from 0 to 10 wat is your level? Please describe me also, the irradiation of pain. See you.
