Where?
1.) Patient not secured to backboard.
Cervical/spine immobilization means securing to backboard.
2.) Pram (stretcher) not locked into floor latch?
3.) Back door not latched/locked ? (or did pram force closed doors open?)
4.) Quick acceleration? Be gentle with patients and crew and vehicle.
What an embarrassment.
