The girl with the cube is Chell - she's the main character in the game - that's who you play as the player. :)

So to control her, they added modules that handled morality and dumbed her down, to keep her from trying to kill the scientists again. But she outlived all of them anyway.

Carolyn was an unwilling subject when put into the computer. As such she freaked out and tried to kill everyone in the facility with a "deadly neurotoxin." In addition they made watching someone complete a test in the testing chambers directly stimulate her pleasure centers... making it an almost insatiable desire to torture people with tests.

GLaDOS is the Artificial Intelligence that runs the Aperture Science testing and production facilities in Portal/Portal2.

She started as a separate AI, and had modules attached to give her specialized abiltiies, etc, but she was initially based on the downloaded brain patterns and memories of Carolun, the assistant to and lover of Cave Johnson, founder of Aperture Science.
