tracheal deviation can be determined by Trail's sign; by inspecting sternomastoid muscle bulging on each side of the trachea. If the muscle is bulging on the right side, then it means that the trachea is deviated to the right side. It can be further confirmed by palpation for traceal tug; which is the distance between the cricothyroid cartilage and suprasternal notch (normally 3 fingers). It is absent in COPD

It is used to determine Tracheal Deviation on trauma patients and people in Respritory Distress to determine which side of the lung is effected.
