The engineer operates two levers in his cockpit. I have noticed that they routinely check their time table (not just glancing, but manually and deliberately pointing at their reference list) to ensure they are on schedule. They then point straight ahead... There is no one else in the cockpit, and unless there is a camera on them, I'm not sure why they do all of these gestures. It is repeated every time they make a stop at a station and sometime while on route.
The reason why they point outside is because they check the signal they are about to pass. It's a way of being perfect in a Japanese society.
