yes theres court, in ohio we go to hte probate court i believe and can ahve it done there. and its about 110$ to file (in ohio) then what ever the court costs are once you have your court date with the judge so he can say ya or nay on ur name change. for the form i can find you a website, but if you just go to google and type in your state then name change you should be ablet o find one
