Interesting how often David uses a closed question or an alternative close "is it a 10 or is it a 5?" type question, when an open one would surely be better. "Are you clear about the direction?" where "where do you see this going" would be much better. Basic error surely?
You could have picked up on her great unconscious communication, which if responded to, would have lead to a more precise elicitation of the real underlying issue and a more compelling outcome. Using her words and gestures would have built deeper rapport, but your client was, as we say "playing nicely".
