I am an ELCA Lutheran from South Carolina. I play the piano and organ. Today (May 24), I played this as the Hymn of the Day. It is #393 in Evangelical Lutheran Worship. Just thought I'd share.

Nice job! Although may be a good idea to speed up the tempo a bit, especially at the end of the piece.
Would like to hear "Come Thou Fount of Every Blessing" by Manz and "Jesus, Still Lead On" by Manz also.

Yes, it's definitely by Paul Manz, from Ten Chorale Improvisations, Set VI Opus 16, published by Concordia, 1975.
