  • Some workers never noticed that the accident happened. If I would'nt have put a L/O-T/O on the panel box they may have turned it on resulting in Electric shock.

  • I once used the Lockout-Tagout procedure, when a forklift snagged a remote to an overhead crane. Resulting in the remote getting ripped of and the wires were exposed. The wires exposed were HIGH VOLTAGE. I located the A/C Panel Box for that crane and switched the power off. Then placed the lockout-Tagout on the handle preventing it from being energized. Then I called for mantinance personel.
