This technique is incorrect. This test is closer to power of external rotation in 90° of abduction in the scapular plane, as described by Walch et al. in the same article as the Hornblower's sign. In the Hornblower's sign, the patient is asked to bring their both arms to her mouth.

@HessuKK Thank you for your comments. The first point we'll make is that is that we are almost finished with a complete overhaul of all our videos that will now include audio and text for use in our apps. During that process we've made some revisions based on user feedback (as we highly solicit on our webpage - peer review keeps the process honest), and this is one of the videos that has already been fixed.

@HessuKK Second, according to Walch 1998, JBJS, even though test gets its name from the image of someone "blowing a horn" that it portrays, a positive sign in that article is specifically described as "if the shoulder cannot be externally rotated in this position, 'hornblower's sign' is said to be present", and that position being resisted external rotation of the forearm in 90 deg of GH ABD in the scapular plane (elbow flexed). In our video the GHJ is more in straight ABD than in scaption.
