@busta4u I know of one, USAir flight 427. The cause of this crash was deemed the likely cause for another United 737 crash.
"The National Transportation Safety Board determines that the probable cause of the USAir flight 427 accident was a loss of control of the airplane resulting from the movement of the rudder surface to its blowdown limit."
The rudder was pushed to it's maximum deflection angle and sheered off, that American A300's rudder was not.

@Adoramus99 No, that's a 777-200. Look at the cockpit windows and the proportions of height to length. Also, note the tailcone. 767-400ER's and all 767's have rounded cones, all 777's have "bladed" cones.
