Every time I watch this, I get this awful feeling that they'll play this at the end of the montage when he dies, and it's going to tear me apart.

Wow. What started out as an absurd comedy bit gradually became extremely depressing. This totally seems like something they'd put in a "death" montage. I'm sad now.

@MikeMartinPhD It wasn't for anything special or anything like that. It was just a comedy sketch they had in the middle of one show around 1998 or so. I seem to remember them cutting to the second guest after commercial break, and Conan made a remark jokingly about them letting him keep the show or something.
