I believe its a 1 time fee of $99 that renews yearly - there is no pay as you go plan.

mobile me is $99 a year - it offers web hosting, email, 20gb of online storage and it integrates with the iLife suite. Its definitely worth it if you have an iPhone. I suggest you check it out.
