@vincentlovedjheinda you need to find the axe and then use it on someone using yellow level execution and then equip by walking over it.

@lDrDro make sure you use the appropriate swing's to chop the head off then walk over it to exchange it with another item you might have on you, just walk over it, then go to the room and put the head over the door opening so the guy can see it, if I remember the PS2 and PSP version you press [o] button but I don't know for PC, if it continues to not work then retry the level.

@lDrDro Search everywhere dude...make sure you execute the first few people in the room, later after that there will be 1 thug coming in at a time look for the Axe up stairs and break the glass equip the axe or exchange it with another melee weapon you already have, then execute someone using the axe by doing it by the yellow level of execution, the red level does not give you the head, or you may take on a thug and attack him head on by using the axe, make sure....
