Ummm Knox said he was gonna take him out...you can't do that while there is an at bat going on you have to wait until before he begins pitching to the next batter...just sayin'

This is a quota, "The precise definition of the ball being dead is a bit confusing, but for the purposes of your question, the ball is dead between pitches, and you can absolutely replace pitchers during an at bat. It's not done that often though." What is more unlikely is to allow any pitcher to get to over 150 pitches.
