Cao Cao was an lower alliance of Yuan Shao all the way until he successfully seized the emperor and defeated Yuan Shu. You can view Cao Cao as a strong sidekick o Yuan Shao. The alliance broke up after both of them become fairly strong.
Yuan family is the most famous and powerful family back then. Yuan Shao has the fame, and Yuan Shu has the money and power. Back then one must take the family reputation into consideration. Cao Cao who has a relatively weak background and power, is not a choice of leader of alliance. Actually, the situation after Doug Zhuo is more like a comflict between two groups: Yuan Shao vs. Yuan Shu. The game has one scenario very similar to the above more realistic setting.

Y'know, you've made me think of something. In DW3 Xtreme, when Cao Cao disobeys Yuan Shao, and his unit gets wiped out, that's why.
