Right after they lock you up at Groznyj Grad, save and reset the game. When you load it, you'll already begin inside the nightmare.

ahh i rember the first time i got this, i was like waite i know i put th right game in as play though i was like maybe my brother bought a new game and i mistucket for mgs3 then it fade to snake waking up. i was fucking luagphing on how awewsome that was.
