I believe extras died during the filming of the sea battle, which actually took place at sea, and in which the ship burnt realistically which it wasn't supposed to do.
There were certainly genuine unscripted accidents during the filming of this race.

@AllyssSings He had a heart attack, which could be very painful. Not as painful as being trampled by a horse and chariot, in my opinion.
Sadly, this wasn't the first or last film that "Breezy" Eason directed. The many horse deaths in 1936's Charge of the Light Brigade led to the American Humane Society observing films to prevent cruelty that we all enjoy today. His six-year-old son died in 1921 due to 'traffic complications' on the set of "The Fox" (although it was directed by Robert Thornby).
