This video is informative and lead me look up what the National Capital Area Medical Simulation Center is. (Hint: Anything including the word 'Capital' is likely to have someone in uniform.)
The exam shown is the way my doctor found my precancerous multi-nodule goiter that was growing all over the place and needed to be removed. It didn't show in the blood work but did get confirmed by an ultrasound.
In my opinion, for suspicious thyroid, employ BPI - Blood work, Palpitation, Imaging.
