Not to be nit picky but...
You can't run Black Magic as demo, even as a pDPS demo lock (really shouldn't be) it's marginal compared to 63 spell power.
Also you weren't refreshing your Improved Shadowbolt on the dummy, that 5% crit is a must to keep up and a gain to keep up.

Domino's for my action bars.
Forte X for the small bar with cooldowns in the bottom-middle, and the timers on the right.
Recount and Sexymap is viseble aswell.
For the Lifetap, Decimation, Eradiction and more "procs" i'm using EventAlert2. Works for more then only warlocks aswell.

Do u see the "wierd" icon as first buff?
It does collect certan bufs and minimizes them if they got more then X minutes duration for example. This is a standard Blizzard® feature in the options menu.
