I don't think Otani was really on bad terms with Mitsunari. Before Sekigahara began, Uesugi Kagekatsu was raising troops. Ieyasu moved to stop him, calling it as a rebellion of sorts. Otani was going to join Ieyasu I think, but Mitsunari stopped him. I don't think the lines of East and West were really drawn at that point.

one big thing that kessen got wrong is the fact that otani yoshitsugu had little love of mitsunari. in fact, before the sekigahara campaign, yoshitsugu was going to join the eastern army, until he was stopped and press ganged into the west by mitsunari ishida. he would of course have regretted this when he was forced to commit seppuku as retainers of kobayakawa hideaki defeated his men

Leprosy is a disease that could causes (among many other serious medical problems) loss of sight, loss of feeling, and a large number of pustules. Yoshitsugu wore his mask because his face was filled with pus. He went almost completely blind towards his last days, and had to rely on his vassals as "Seeing-Eye" warriors.
