Early trial version of Perfect Chicken Flavor (2nd Touhou Ronald Project).

There is no donatic difficulty yet, it's still work in progress. The extra stage is called "Order of smile" and it looks promising so far. I don't expect it to come out soon... The creator said that it would take 8 months to complete.
