He starts out playing the riff to 'Motherless Children' & then sings 'You Bout to Take Sick & Die.' Blind Willie Johnson & Muddy Waters & many others. One of Blind Lemon Jefferson's first recordings 'You're Gonna Need That Pure Religion, Hallelujah' is an early version of this tune from 1925.
I'm pretty sure this is E standard tuning.

i wonder what tuning is he in?

this song is bone chillin ,its cold but it fair! and i must say its the truth. but the best part is the way he make that guitar talk, thats that soulful oldschool chops! the kind that you can't help your self by moving to the groove. i can play this for the pope and he'll strart clappin & stumpin his feet to the beat.

Amazing to hear the little story at the end, made my hair stand on end and my eyes well up.
