Actually Hashem tells us at the end of Exodus when instructing Moshe and Aaron on setting up the Tabernacle and how to Worship that we must bath our selves in a Mikvah before entering G-d's house of worship. It should also be mentioned in the midrash.
shalom! on the rosh, make sure it is above your natural hairline, and on the soft part above your neck, on the back of your head, that is where the knot should be. You can find great helpful chabad videos on youtube so you can get it excactly right. Don't feel intimidated if they wrap it differentley than you, there are many different customs. Make sure you get it right, and do it every day.
Not that I would lay tefillin while I am dirty, but can you explain where in Torah it refers to one's personal hygiene when it provides instruction on tefillin.
Sorry, but in general, I hear a lot of stuff that seems to have no basis in Torah..... Yes, it makes good sense to be physically clean when laying tefillin. However "good sense" is different from being a commandment.
All too often, people add things to His laws because it feels right to them, personally. That's when Judaism strays.

the non.chassidic ashkenazi custom doesn't make a shin around the bicep. the shin is on the hand.
