It looks like the person who did the end credits had a little too much New Year's egg nog (this classic episode aired on New Year's Day 2003). According to this clip, Colleen Zenk Pinter plays "Barbra Ryan" and "Martha Burns" plays Lily.

This was a great episode, by the way. It's a shame CBS can't be bothered to show classic ATWT episodes anymore.
