@dongisselbeck puffed cheeks are usually due to a nerve defect or injury in the player's cheeks/face, or possibly an effect of trying to play a bad harmonic in tune.
What IS that instrument the woman is playing at 2:40? It looks and sounds like maybe it's some constipated predecessor of the oboe.
