NOTE: This does not apply to iOS4. It works on all previous iOS versions as far as we know...
iPad USERS: This does NOT work on an iPad.
Remember: Fill 1st & last pages with apps; have at least one app on all other pages; put one of the last alphabet Apple app in the last spot on the last page (like Voice Memo or Youtube); force that app off by dragging another app to the page (easy to do with an app from your bottom row if you want); turn your device off; turn it back on. Do you have a new 2nd page with only one app on it? If so, you've done it!
Judging from the iOS4's 'folders' concept each with 12 apps, the max install base will be 2,160 apps. Well, that translates to 135 pages of apps at 16 each for the iOS3.x using this technoque. I've just added page 57 (not all are full, but I've got 'categories' per page -- sort of like folders -- but with up to 16 rather than 12). Has anybody hit a limit? I'm on an old 8GB 2G original iPhone, mind you, running iOS3.1.3. Your results should be even better...

BACKGROUND:
In iOS 3 for the iPhone/iPad Touch, Apple has limited us to 11 pages for holding application icons. Apps added after all 11 pages have been filled up do not appear on the phone (even though they are installed on the phone). This tip from a listener to the "Today in iPhone" Podcast reveals a work around to this limitation. It does not require jailbreaking your phone, and has been used here to add a total of 19 pages of apps. This video reveals an attempt to create a 20th separate page, surpassing Apple's ability to show the pages with the 'dots' across the bottom of the screen. Will it work or blow the phone? Watch and see.

UGH! It deleted like 50 of my apps because of 9.1!

@3greviews OH NO!!! This could be real bad news. Thanks for posting this. I was just about to update. I'm going to stay at 9.0.3 for now. Can other viewers please post here if you have the same experience with iTunes 9.1 or if your pages remain intact after you upgrade and Sync. Thanks you for helping all of us decide what to do.

@magicaliphone Just did my upgrade expecting the worst, and everything went smoothly. I'd suggest taking a picture of each of your pages with the apps where you want them -- just in case you need to recreate your pages -- then bully backup your phone, then update iTunes, reconnect, and see what you've got. If everything is fine, do another backup. All 57 of my pages came through just fine....
