@IamAeroDynamic I keep rewatching from 0:03 and this is my guess:

He's in a huge gear. He's driving down with his left leg when the gear slips a bit. But he's off the saddle with his weight way forward and sort of falls over the bars. Note how his armrests almost catch him in the gut. He can't recover and tumbles over.
