People seem to be getting confused with this...
in order to promote the SAW films, they have held a blood drive each year. the concept of the nurse is because obviously when you think of giving blood you think of a nurse, and this is giving the nurse a Jigsaw twist; hence the dress made out of needles (remember the pit of needles in SAW 2). and notice the mask over her face in the final photograph, similar to the promotional posters of SAW 5.
it's not that difficult to understand guys.
