I think the commercial makes it obviously clear that the guy is an acupuncture sham, seeing how he gives treatment in his filthy apartment and has poor personal hygiene even while treating a patient.

Just to clear things out here, this is a commercial for a Dutch insurance company called Apeldoorn. This is a pretty old commercial, and I'm seeing a lot of comments regarding Final Destination 5, this commercial is older than that...
The "even Apeldoorn bellen" text at the end means "Just Call Apeldoorn"

@ernestezekiel its and ad for 'Centraal beheer Achmea' which is established in Apeldoorn so the text at the end said "Just call Apeldoorn"
