The way I interpret it is that David is illustrating a person who goes through life indulging their own pleasures (by means of drinking in excess, getting high, etc.) and in the process loses the memory of what has happened and how they got where they did. The person feels like they're in a foreign land after waking up from being blinded by their self-indulgence. The reference to water underground, I think, is symbolic of the state of being high, as in how it feels like you're underwater.
