Speed Over Beethoven from Dance Dance Revolution Extreme 2 -- DDR Drum Track

The layout is exactly like Rock Band drums. The first 4 arrows are the red, yellow, blue, and green pad. The 5th arrow is unused, and the 6th represents the bass pedal. I know it looks complicated, but it's really easy once you get used to it.
It's on auto-play because I didn't feel like playing the drums when I recorded this.
