i'm going to say because whenever you take a bite, you have to feel yourself swallow the food, which is more pronounced when somthing's wrapped tightly round your neck, and it gives you guilt. The more you are aware of your guilt and your eating the less likely you are to eat.
Otherwise it could symbolize choking, perhaps the feeling that she is being suffocated by her sisters disease?
