Before you can shoot the injection tubes, Harbinger and the last few defenses try to stop you. After a brief firefight, you fire on all four tubes, with the Reaper falling to the chasm below.
As you're about to set the charges, the Illusive Man makes contact. He's glad you made it, but offers you an idea: save the station, and all the tech could be used to possibly defeat the Reapers.
Knowing that that kind of info is too much to put into anyone's hands. No matter what, the base must be destroyed.
Before you can leave, you're rudely interrupted by the appearance of the human Reaper...it's not dead and only you can take it down.
With the Reaper destroyed, you are thrown all over the place. When you wake, both Miranda and Jacob are battered, but fine.
As you run, Harbinger warns you that this isn't over. Racing out of the base, the Collector ship blows up taking everything with it...
Afterwards, you have a final 'chat' with the Illusive Man. You part ways...but something tells you this isn't the last you'll hear of him.
While the Normandy is being repaired, Joker hands you a data pad with info taken from the Collector Base...it shows Harbinger.
As you look out across the galaxy, you imagine the sheer horde of Reapers slowly making their way towards the final confrontation...

I'm surprised you didn't use the M920 Cain(aka the nuke) on the Reaper at the end.
