I brush my dogs' teeth every night. It's not necessary to train your dog to lie down like this for the tooth-brushing; it can be done while they're standing, sitting, or lying down. And yes, you use special pet toothpaste, not human toothpaste.
Dogs initially aren't too happy about this, but they get used to it and either put up with it, or actually come to like it. They do like the taste - it comes in mint, chicken, or other flavors. Yum! Meat-flavored toothpaste.
