@AlienBusDriver first of all, they can barely afford to eat. Second the local government rarely give out passports. Second, after Mao's "era" in China, people are afraid of the government.

@AlienBusDriver your friend is talking about the poor villages all over china (Western & Eastern China) Living conditions are very bad, heath care and basic supplies sometimes don't even exist. If you drive 30~40km out of Beijing you can find villages like this. Also china have this "registered residence" policy. You are divided into 2 types of people by where you were born. So if you live in a village, you don't get anything from the government and you cannot leave the village to live-

@Whatsup1492 what is it like, my friend tells me the common people dont even use toilets, they just piss in bins, this sounds very far removed from the west which I am use too. I have a toilet, I have welfare if Im sick China I guess doesnt offer this.
