The war2 deathknights were not the same as War3, Guldan reanimated the corpses of slain stormwind knights and infused them with the souls of dead orc warlocks , Teron Gorefiend was formerly a head walock in the Orc shadow council and served Nerzhul when he was Warchief, Obviously he didn't escape dreanor with nerzhul when it blew up and was traped in outland.
